Multimedia Fees

To pay for a multimedia fee (late fees ,repair fees, loss fees, etc…), please fill in the following form.

If this payment is destined to another person than you, please enter the NetID of the person the payment is targetted to. A student NetID is composed of the letter 'a' followed by the student ID number (eg. a12345)
